Returning Home from Rehab at Shannondell
It is our goal to make your transition back home as seamless as possible, and we will work with you and your family to ensure a smooth adjustment.
Very early in your stay with us, a member of our social service team will meet with you to discuss your living arrangements and your plans for returning home. We will focus on understanding your goals and make sure that everything you need to return home is available to you.
As a service to you, before you leave Rehab at Shannondell we will provide you with a discharge wellness organizer to help you keep track of your post-discharge prescriptions, equipment, appointments and personal medical data. Using this organizer will help you centralize all of your rehab-related papers and documents in one place for easy access.
Prior to the day of your discharge, we will work with you to ensure that you have adequate transportation for your trip back home. If family members are involved in your care, we will coordinate with them and assist them in planning for your return home.
Rehab at Shannondell accepts most major insurance plans. Many require pre-certification prior to admission, and most cover medically necessary rehab services. But because each plan is different, it’s wise to discuss your specific circumstances with your insurer to determine any co-pays, deductibles and out-of-pocket expenses.

Your progress at Shannondell will be well documented and included in your discharge wellness organizer. It’s important that you share this documentation with any medical care professionals you visit during your continued recovery after you are discharged and continue your therapy program. This includes your primary care physician, cardiologist, orthopaedist, home health professional or outpatient therapist.
To ensure your progress, our licensed therapists may recommend that you continue with your therapy through an outpatient clinic or home health service — or here at Rehab at Shannondell on an outpatient basis. We will coordinate these services for you and assist you in selecting the best provider for your needs. In addition, if you need any medical equipment at home, such as a walker, cane or other assistive device, we will order it for you prior to your discharge so that it is available for you at home.
