How do I get referred to Rehab at Shannondell as a hospital patient?

If you are in the hospital, please contact the person that is managing your care (typically a Social Worker or Case Manager) and let them know that you prefer Rehab at Shannondell for your rehab and recovery.

Can I tour the facility, observe therapy space and talk to the staff?

Insurance

What types of insurance are accepted at Rehab at Shannondell?

Rehab at Shannondell accepts most major insurance plans. Before you arrive, please call a member of our business office staff to discuss your specific coverage.

How do I find out what my insurance covers and what I might be responsible for paying?

Upon admission, a member of our business office staff will meet with you to discuss your insurance coverage and provide details about your out of pocket costs. If you have questions prior to admission, please contact us directly.

Therapy

What is an individualized rehab plan?

Our therapy team will develop a comprehensive treatment plan that is specific to your individual diagnosis, capabilities and goals.

How often will I receive therapy?

Our therapy gyms at Rehab at Shannondell are open 7 days a week. The frequency of your therapy sessions will be determined by your diagnosis, capabilities and insurance coverage.

Are my personal Doctors informed about my progress in therapy?

Our team will be glad to communicate with any physician of your choice. You will also be provided with a detailed summary of your therapy accomplishments upon discharge to share with your personal physicians and specialists.

How long will I need rehab?

The length of your stay at Rehab at Shannondell depends on your individual condition, motivation and stamina. Part of our medical team’s initial assessment with you will be setting expectations for recovery goals, including timeframes for your return home. With this said, the decision as to when you’re ready to return home is dependent on both your progress and confidence of our team’s assessment that you’re strong enough to be discharged.

What if I want to continue therapy after my stay at Rehab at Shannondell?

Rehab at Shannondell offers an outpatient therapy clinic – including physical therapy, occupational therapy and speech therapy – within the Shannondell at Valley Forge community. Our discharge team will be glad to coordinate this for you.

Daily Life

Where will I eat my meals and how would you describe the food?

We encourage our residents to dine in one of our table service dining rooms or bistros. If you prefer, room service is also available. Regarding the quality of meals, we refer you to our patient stories. Several speak to the pride that our chef, kitchen staff and servers take in the variety and quality of food at Rehab at Shannondell. Simply stated, you’ll enjoy your meals here.

Should I bring my laptop, tablet or other mobile device with me?

Free Wireless internet is provided throughout the Rehab at Shannondell facility. You are encouraged to bring whatever device you feel would be helpful in maintaining contact with friends, family and co-workers.

What kind of clothing should I bring with me?

You should plan to bring comfortable clothing that you would exercise in at home.

Medical Care

Who will be my Doctor while I’m at Rehab at Shannondell?

You will be assigned a primary physician prior to your arrival at Rehab at Shannondell. These physicians will coordinate your care with any external physician that you choose.

How often will my Doctor visit me?

Rehab at Shannondell physicians are available in person, Monday through Friday. Our staff physician and/or our nurse practitioner will visit you as often as is necessary during your stay. Should it become necessary to discuss your care with one of our physicians during the weekend, one of our registered nurses will contact them.

What other clinical professionals are available to me?

The clinical management team at Rehab at Shannondell is made up of a staff physician, a full-time nurse practitioner, registered nurses, a registered dietician, licensed therapists and certified nursing assistants.

What if I need emergency care while I’m at Rehab at Shannondell?

If your physician determines that your specific needs require emergency treatment, you will be promptly transported to an acute care hospital.

Can I have visitors during my stay at Rehab at Shannondell?

Visitation is an important part of your recovery. At Rehab at Shannondell, there are no restrictions on visiting hours. For overnight guests, we can recommend several nearby lodging options.

Can anyone join me for meals?

Yes, we encourage friends and family members to join our residents at meal times. Please contact the concierge at the main reception area to arrange for guest meal service.

Can my family observe my therapy sessions?

Yes, family members can observe your therapy sessions.

Are there nearby overnight facilities available for out of town family or guests?

Yes. Out of town guests will find abundant lodging options nearby. Please contact the concierge at the main reception area for details.

Discharge

How will I be discharged from Rehab?

Our discharge team – made up of social workers, physicians and therapists – will communicate with you from early on regarding your needs at discharge. Your transition back into your home environment is critical to your ability to remain well after you’ve recovered well at Rehab at Shannondell.
